New Day Healthcare is currently seeking a Community Care Specialist in the Joplin, MO area.
Pay rate: $18-20 per hr
Full-time-Monday-Friday 8-5 p.
m.
Responsibilities:
Receive incoming calls and messages from potential clients and consistently strive for emotional linkage and developing relationships.
Determine client needs and assist in coordinating care by referring to appropriate service lines.
Collect, share, and manage information using respective databases.
Clearly communicate detailed information to appropriate teams.
Track referrals, keep status and notes up to date in database, and advocate for patient care.
Internally build relationships for the furthering of serving clients.
Make outgoing calls to patients in the care continuum.
Performs other related duties as directed by the Supervisor.
Qualifications, skills and abilities:
Patient, attentive and empathetic listener.
Must have strong phone etiquette with the ability to communicate well, both verbally and in writing.
Must be detailed-oriented, accurate, organized, multi-tasker, and great with time-management.
Initiative-driven individual able to work independently and in a group setting in a fast-paced environment.
Computer proficiency and knowledge of Microsoft products.
Must be reliable, self-starter, strong work ethic and work well within a team.
Previous sales or marketing experience preferred.
Experience in healthcare preferred.
Will train the right candidate.
