*POSITION TITLE:* Case Manager I *DEFINITION:* This is a beginning-level professional case management position working with individuals who have developmental disabilities.
An employee in this class is responsible for managing a caseload of moderate size and complexity.
Case Management at Developmental Disabilities Resource Board of Jasper County/Connections Abilities Resource consists of connecting, planning for services, linking individuals to services, and advocating for service improvements for all persons served.
As a member of an interdisciplinary team, the employee participates and confers with other professionals in determining the proper treatment to purchase or procure for persons served.
This position is a sensitive position as it works closely with or around individuals with developmental disabilities and will require an annual criminal background check.
Close supervision is received from a Program Coordinator.

* *EXAMPLES OF WORK* Ø Manages a caseload of moderate size and complexity, normally involving a variety of developmental disability areas.
Ø Demonstrates that the person’s served/family’s ideals and values are understood and will be respected.
Ø Interviews person served, their families, and others involved with the person served; assists in completing application for resources; collects basic data and obtains appropriate additional information from other agencies.
Ø Prepares case notes, summaries, and other reports concerning the person’s served case.
Ø Develops, in cooperation with inter-disciplinary team, an Individual Service Plan (ISP) for each person served.
Ø Attends inter-disciplinary team meetings to discuss the persons served in relation to service plans, progress, and possible changes in the service program.
Ø Monitors services to ensure that the terms of the authorization are being fulfilled by the other agencies, to check on quality of services, and to review progress as required and must be in compliance with all local, state and federal authorities.
Ø Provide documentation to account for indirect work time (this is time worked that cannot be logged to a specific individual; i.
e.
committee meetings, workshops, etc.
) Ø Provide progress reports and guidance to the person served, their families, and other responsible individuals.
Ø Attend staff meetings and review new or revised policies and procedures.
Ø Attend and participate in meetings with a CORE group of case managers in order to share ideas and provide support to colleagues.
Ø Visits service agencies, attends meetings, and confers with other Case Managers to become informed concerning community resources for persons with developmental disabilities.
Ø Reviews and applies all documents, training guides, and training and confers with other Case Managers concerning theories and practices in the fields of Human Services for persons with developmental disabilities.
Ø Meet or exceed logging target goal set by the organization.
Ø Logging is to be maintained within mandated 72 hour of date of service Ø Complete all service authorizations & budgets within the specified timelines.
Ø Keep all information systems current; to include but not limited to CIMOR, MoEDI Web, Jarvis, and any other database.
Ø Prepares and maintains accurate expenditure records.
Ø Receives close supervision from a Program Coordinator.
Ø Position could at times require over 50% travel within Jasper County & occasional travel outside of Jasper County; which requires reliable transportation.
Ø Performs other related work as assigned.
*KNOWLEDGE, SKILLS AND ABILITIES (KSAs)* Ø Knowledge of a specific discipline such as social work, psychology, special education, counseling, health care, or occupational therapy.
Ø Introductory knowledge of case management methods, principles, and techniques.
Ø Introductory knowledge of various developmental disabilities and corresponding agencies and services available for clients.
Ø Introductory knowledge of interviewing methods, principles, and techniques.
Ø Ability to manage a caseload of clients with developmental disabilities, to keep service plans current, and to maintain accurate records.
Ø Ability to collect and analyze information to make decisions concerning a person’s served service plan.
Ø Ability to develop an effective practical service plan for all persons with developmental disabilities.
Ø Ability to communicate effectively.
*EXPERIENCE AND EDUCATION * A Bachelor's degree from an accredited college or university is required.
